Referring Programs may click on the link below to review and print a copy of our Eligibility Enrollment Applicaiton.  NOTICE:  Eligibility Applications will ONLY be accepted from Referring Providers and NOT individuals.

REFERRING PROGRAMS

Pre-Dental Students & Dental Assistants wishing to become a volunteer with Wake Smiles may click on the link below to review and print a copy of our volunteer application (If you do not see the appropriate catagory for your volunteer status you may write it in on the application).  You must return the completed application with a check made out to Wake Smiles in the amount of $25 to 2620 New Bern Ave in Raleigh on any Tuesday evening between 5pm and 8pm.  Your check will be returned un-cashed once your background check has been completed AND you volunteer. 

Notice:  If you have NOT completed an OSHA infection control class you may shadow and set up clean oporatories.  If you have completed an OSHA infection control class, please bring proof of completion to volunteer in additional capacities. 

Additional information on OSHA classes are posted in the Wake Smiles office. 

VOLUNTEER APPLICATION

without completing the volunteer application.
