Wake Smiles Patients

       Thank you for your interest in becoming a

Wake Smiles clinic patient.

To become a new Wake Smiles patient please follow the steps #1 - #3 below.  If you are an existing Wake Smiles patient and your eligibility forms have already been recieved please follow step #3 to schedule your next appointment.

#1. Visit the following program to fill out eligibility forms .

  -Wake Health Services (919) 250-2930  (2620 New Bern Ave. in Raleigh)

WHSI is open Monday thru Thursdays 8am - 5pm

Horizon, Urban Ministries Open Door Clinic, The Raleigh Rescue Mission and The Healing Place only refer current patients and residents. 

#2. Once you are approved to participate with the Wake Smiles program ask your referring provider or program to forward your eligibility information to Wake Smiles.

#3. Once your eligibility information has been recieved you may schedule yourself an appointment with Wake Smiles. Appointments cannot be made for individuals whose eligibility forms have not been received or whose eligibility has expired. To Schedule an appointment please follow the below instructions:  

          You will need to call Wake Smiles appointment line on Tuesday between 1pm and 2pm to schedule your appointment.

To Make Appointments For Wake Smiles!

All calls will be taken in the order in which they are received and no calls will be returned , so please do not leave a message for appointments. If you do not speak to the coordinator continue calling until you do or until 2pm. In the event you do not get through to make your appointment, please follow the same instructions for the next Tuesday. Once you have an appointment you must keep that appointment.  If you fail to show for your appointment you will not be allowed to schedule future appointments.

NOTICE:

To ensure equal access to all patients, appointments will NOT be made for individuals who show up at the clinic on Tuesday mornings or Fridays, Please follow the above guidelines!

Calling to get your initial appointment may take more than one appointment calling session as Wake Smiles has over 1400 patients and approximately 15 appointment slots per week.  Thank you in advance for your patience and understanding.
